Submit Documentation
Upload or mail in a copy of your sales receipt and proof of installation.
Submit Your Rebate
BY MAIL
Send in your original dated store invoice, completed rebate form from the store, and your completed warranty card that is included with your product to receive your rebate. A blank rebate form and warranty card are available below if you did not receive them at purchase.
